Waikiki Health is seeking a full-time Community Health Worker
Pay rate for this role is $22.00 per hour
Schedule: Monday – Friday 40-hour work week

The Community Health Worker (CHW) is responsible to assist in improving outcomes for high-risk patients, who utilize the ED/Hospital regularly and/or that have been identified using our Social Determinants of Health (SDOH) screening tool, PRAPARE. This position will help achieve better outcomes, improved patient experience, and improved provider experiences. The CHW will assist in providing services during clinic hours, participating in huddles, population-based management, patient intakes, education, and counseling, within clinic protocols.
Qualifications and abilities
● Successful completion of a Community Health Worker program or BA in Public Health is preferable.
● Three years of experience in an outpatient setting preferred.
● Transportation and valid driverÂ's license.
● Familiarity with common software programs, (i.e. Windows-based programs) an ability to quickly learn new systems.
● Ability to communicate effectively with patients, community members, and staff members.
● Proficient in computer skills: typing, navigating through the internet and emails.
● OSHA training.
● BCLS biannually.
General Duties include but are not limited to
● The CHW is a patient advocate and facilitator of the Patient Centered Medical Home (PCMH) and its target population of high-risk SDOH patients and high ER/Hospital utilizers through the provision of health education, follow-up monitoring, and improvement of access to care.
● Develops and implements a Care Plan tailored to the needs of the patient and the patientÂ's immediate support system. Through this patient centered approach, the goal of this relationship with the patient is to decrease health disparities and improve health outcomes, while also assessing and addressing the patientÂ's social determinants of health (SDOH).
● Collaborates with multiple WH providers, staff members, healthcare data analyst, insurance agencies, partner agencies, and diverse patient populations.
● Documents care in EMR vital statistics and other pertinent data of patient per clinic procedures accurately. Must be able to spell words correctly, both in English and medical terminology. Maintains strict patient confidentiality.
● Other duties as assigned
